Hospice Care In a Skilled Nursing Facility
In-House Hospice & Palliative Care provides services wherever the patient is living or receiving care. In some cases, this means that a patient's primary residence is a nursing facility. We partner with nursing home staff and work with patients, families, and their care providers to assure a quality continuum of end-of-life care.
Working closely with the patient, family, and staff, the hospice team provides comprehensive care that addresses the physical, emotional, and spiritual concerns that can arise when a person is at the end of life. Patients living in nursing homes receive all of the same services provided to home care patients and their families. In-House Hospice & Palliative Care oversees each patient's plan of care, but relies on the nursing home staff to do what family members would normally do in a home setting.
When we provide hospice care in a facility, we consider the staff at the facility as part of the caregiving team and, when needed, we provide training in end-of-life care to each nursing facility.
Hospice care is being used more frequently to provide skills and services that are not otherwise available in nursing homes. The services of the hospice team supplement the usual nursing home care at a time when staff, family members, and the patient are facing the increased and urgent needs associated with the dying process.
Our professional staff works closely with the nursing home team to create a positive impact on end-of-life care for residents that, in turn, improves quality of life. The nursing facility staff is always able to speak with our nurse 24 hours a day, seven days a week.
