Hospice Care In an Assisted Living Residence
In-House Hospice & Palliative Care employs physicians, registered nurses, social workers, chaplains, certified nursing assistants, volunteers, and bereavement counselors to deliver an integrated, holistic approach to health care, no matter where that person may reside. We have partnership relationships with most assisted living facilities and group homes in our service areas. Special services are available to meet the unique needs of seriously ill patients in assisted living facilities and to complement care the facility provides.
We work as partners with our community's assisted living facilities to ensure that all aspects of the patient's end-of-life health care needs are being met. As always, our staff works closely with the patient, family members, family physician, and the assisted living facilities or group home staff to address each situation as a special opportunity to provide needed services. In assisted living facilities, hospice staff does not take over patient care, but rather combines their skills and expertise with facility staff.
A registered nurse provides care and also coordinates care with the facility staff and with the other members of the In-House Hospice & Palliative Care team. Social workers, chaplains, home health aides, volunteers, and other therapists are available to visit the patient and family and provide support. Art and music therapists, plus a child/teen specialist, are also available and can be especially helpful if there are children or grandchildren in the family.
