Hospice Care In a Hospital

In-House Hospice & Palliative Care has a partnership relationship with numerous hospitals in our service areas. When a loved one in the hospital is facing a life-limiting illness, hospice services can be initiated before they leave. We work with patients, families, and their care providers to assure a seamless continuum of quality end-of-life care. Working closely with the patient, family, and staff, the In-House Hospice team offers comprehensive support that addresses the physical, emotional, and spiritual concerns that can arise when a person is at the end of life.

When we provide hospice care in a hospital, we work with the facility's staff to establish a collaborative plan of care. We consider hospital staff part of the caregiving team and provide training in end-of-life care, based on the need of each facility.

And when the patient is discharged from the hospital, our staff will assist in the transition to the new location and continue to provide services.
