• Our experience -- In-House Hospice & Palliative Care is one of the leading hospice providers in Michigan and Ohio serving nearly 500 patients and their families every day in more than 44 counties in Michigan and Ohio.
• Our commitment to care -- In-House Hospice & Palliative Care's mission is to provide the highest level of quality care to its patients. Our caring and compassionate team strives to deliver the highest level of care available, including pain and symptom management. We tailor our dignified care to the needs of the patient and his or her loved ones. We are available 24 hours a day, seven days a week for our patients and families, and for admissions.
• Our team approach to care -- In-House Hospice & Palliative Care's interdisciplinary team of caregivers creates an individualized plan of care for each patient. The team includes: Patient and Family; Personal/Attending Physician; Registered Nurses, LPNs and Certified Nurse Aides; Social Workers; Chaplains, Rabbi and spiritual care advisors; Dietitians; Pharmacists; Physical, Speech and Occupational Therapists; Massage Therapists; and Trained Volunteers. Each In-House team makes three to five weekly visits to the patient, with additional visits after hours, if needed. Solid communication tools ensure that the plan of care is adhered to.
